COMPLETING THE BADGE APPLICATION. Complete the form in accordance with the instructions on page 3 of the application and obtain the necessary signatures. Please remember these basic instructions when completing the application: |

You may complete all fields in the EMPLOYEE section of the form online, then print, sign and date the form under Employee Signature and under Applicant's Signature. |

b. |
If you do not complete the EMPLOYEE section online, you must print legibly and complete all fields in the EMPLOYEE section using black or dark blue ink only, then sign and date the form under Employee Signature and under Applicant's Signature. |

c. |
Please note that all correction tapes/fluids are prohibited (if you make a mistake, please complete a new form) and that applications are not to be folded or to be copied double sided (front and back). |

d. |
Upon completion of the EMPLOYEE section, take the application to your employer for a duly authorized agent of the company to complete the EMPLOYER section, including signature and date of signature. Please note that if your company is new to DFW Airport, a Company Profile form must be completed and returned to the ACO prior to employees beginning the badging process. |

e. |
Upon completion of the EMPLOYER section, take the application to an authorized badge sponsor for signature, date of signature, designation of badge color, expiration and access matrixes. |
| |
f. |
Applications must be presented to the ACO within thirty (30) days from the date of the authorized tenant airline, governmental agency or Airport Board badge sponsor's signature and will not be accepted after the thirty (30) day period. |

g. |
Bring the completed application to the ACO along with the following required identification when you are fingerprinted and again when you return to be photographed for the badge: |

i. |
A valid government issued photo ID from List A or List B on the List of Acceptable Documents; |
| |
|
ii. |
If your valid government issued photo ID is from List B, you must also bring a document that is on List C on the List of Acceptable Documents; and |
| |
|
iii. |
A document containing your name and street address of your current place of residence (ex. electric bill, water bill, credit card statement, etc.). Please note that if you use your driver's license as your valid government issued photo ID and it contains your current street address, you do not have to provide another document to verify your current place of residence. |

U.S. DEPARTMENT OF HOMELAND SECURITY – CUSTOMS & BORDER PROTECTION SECURITY ACCESS CLEARANCE. If your job responsibility requires you to access a Federal Inspection Services (FIS) area, aircraft deplaning and ramp area, and other restricted areas designated by the Customs & Border Protection (CBP) Port Director, additional clearance is required by CBP. You will need to complete CBP Form 3078 and present it to CBP along with a letter on company letterhead that attests that a background check was conducted on the applicant to the extent allowable by law. The letter must also contain a description of the duties that will be performed in the CBP area. CBP paperwork is accepted at Terminal D on the Arrivals Level in Room D29L118. COMPLETING THE FINGERPRINT APPLICATION. First time applicants are required to clear a digital electronic fingerprint-based Criminal History Records Check (CHRC) before a badge can be issued. Download and print the Fingerprint Application. Applicants are required to read and sign a Fingerprint Application before receiving fingerprint services. This application lists the 28 crimes that disqualify applicants from receiving a badge. It also advises the applicant of her/her responsibility to self-disclose any arrests/convictions received while possessing security access privileges with DFW Airport. Please note that you must bring the required identification (see the list above under number 2 g), badge application and fingerprint application with you when you come to the ACO to be fingerprinted. |

PRESENTING YOUR APPLICATION TO THE ACO. Bring the items listed below to the ACO, where you will turn in your applications and be fingerprinted in order to start the badging process. |

a. |
Completed badge application, including all required signatures; |
| |
b. |
Completed fingerprint application, including all required signatures; |
| |
c. |
The required personal identification (see the list above under number 2 g); and |
| |
d. |
The certification letter if the applicant is sponsored by an airline. |
| |
Generally, the CHRC results are returned within one (1) to three (3) business days and the Transportation Security Administration (TSA) Security Threat Assessment (STA) results are returned anywhere from four (4) business days up to two (2) weeks. Airport-sponsored applicants will be notified once clearance is received on the CHRC and the STA. Applicants have thirty (30) days from the notification date to return to the ACO and claim the badge. Airline-sponsored applicants will be notified of the CHRC clearance through the carrier and of the STA clearance by the Airport. Airline employees will be provided a certification letter to bring to the ACO authorizing badge issuance. The certification letters must be presented to the ACO within thirty (30) days from the date of the letter. Failure to return within these timeframes will cause the application to expire, forcing employees to re-apply and start the process over from the beginning. |

SECURITY TRAINING. Applicants applying for SIDA badges will be required to submit to security training after they have been cleared of the 28 disqualifying offenses listed on the Fingerprint Application, and before the badge issuance. Security training is administered in the ACO as follows: |

Monday through Thursday: 8:30 am, 10:00 am, 1:30 pm and 3:00 pm |

b. |
Friday: 8:30 am and 10:00 am |
| 7. |
BADGE RENEWALS. To renew your badge, complete a badge application according to the instructions listed above in numbers 1 and 2. Badge holders are required to renew access privileges within thirty (30) days of badge expiration. Although expired badges will be denied access when the badge is presented to a badge reader, employees are given an additional thirty (30) days to renew the badge without penalty. Failure to renew within thirty (30) days from the date of badge expiration, shall require the applicant to complete the badging process from the beginning, including the fingerprinting process. |
