Supplemental Badge Application

Additional information is now required to process your application for a DFW Airport Identification/Access Badge.
COMPLETING THE SUPPLEMENTAL APPLICATION
1) You may download and complete the DFW Airport Supplemental Identification/Access Badge Application online, then print the form and bring it, along with your DFW Airport Access Badge Application, to the Access Control Office. Please make sure you complete all applicable fields.
2) If you do not complete the DFW Airport Supplemental Identification/Access Badge Application online, then you must print legibly and complete all applicable fields using black or dark blue ink only and bring the form along with your DFW Airport Access Badge Application to the Access Control Office.
3) Please note that all correction tapes/fluids are prohibited and that applications are not to be folded or to be copied double-sided.
4) Please also note that you must bring your proof of identity and proof of employment eligibility documents from the List of Acceptable Documents with you when you come to the Access Control Office.
